Admissions - Immunization Requirements Lutheran School Of Nursing
Measles (Rubeola) Documentation of one of these:
- Two doses of live measles vaccine separated by at least one month on or after the first birthday OR
- Laboratory evidence of immunity by providing a copy of a positive measles titer OR
- MMR (measles, mumps, rubella combined) vaccination*
- Adults born prior to 1957 are considered immune
German Measles (Rubella) Documentation of one of these:
- One dose of live Rubella on or after the first birthday OR
- Laboratory evidence of immunity by providing a copy of a positive Rubella titer OR
- MMR (measles, mumps, rubella combined) vaccination*
- Adults born prior to 1957 are considered immune, except pregnant women
Mumps (Parotitis) Documentation of one of these:
- One dose of live mumps on or after the first birthday OR
- Laboratory evidence of immunity by providing a copy of a positive mumps titer OR
- MMR (measles, mumps, rubella combined) vaccination*
*MMR: one dose at 12-15 months of age or later and second dose at 4-6 years of age.
- Adults born prior to 1957 are considered immune
Diphtheria/Tetanus Documentation of:
- A booster given within the past 8 years.
Chicken Pox (Varicella) Documentation of one of these:
- Two doses of Varicella vaccine separated by at least one month OR
- Laboratory evidence of immunity by providing a copy of a positive Varicella titer
Hepatitis A Documentation of:
- Two doses of Hepatitis A vaccine given 6 months apart OR
- Laboratory evidence of immunity by providing a copy of a positive Hepatitis A titer
Hepatitis B Documentation of:
- Three doses of Hepatitis B vaccine: Dose #2 should be 1-2 months later and #3 should be 4-6 months after the first dose OR
- Laboratory evidence of immunity by providing a copy of a positive Hepatitis B titer
Required for all students:
- Influenza: Documentation of annual influenza vaccination
- PPD: A PPD test administered at LSN is required to be completed during the first days of attendance of Nursing Track classes then again before beginning Level II classes. There is a fee for this and it is mandatory. PPD test results are not accepted from any other source. Those students who have previously positive results must provide a negative chest x-ray result within the past 2 years.
Required for all students living in the LSN dormitory
- Meningococcal: Documentation of vaccination within the past 5 years
Recommended for all students to prevent disruption in academic activities
- Influenza: Documentation of annual influenza vaccination
In addition to the above vaccination requirements, all LSN nursing students must have completed and submit a current physical exam
within the past 6-12 months and the “Student Medical History” form. A PPD test is required to be completed during the first days of attendance of Nursing Track classes and again before beginning Level II classes. There is a fee for this and will be mandatory.
All students MUST document compliance with ALL requirements prior to entering the nursing program or beginning residence in the dormitory. A minimum of the 1st dose of Hepatitis A & B and/or varicella vaccine must be complete. If documentation is not complete, the student will be unable to participate in required clinical laboratory experiences or reside in the Nurses Residence.
All documentation is to be turned in during Orientation, including records of titers. It is STRONGLY recommended that you keep copies of all documentation in your own files.
